We recommend that you contact your insurance company to verify that we are in-network with your specific plan. In general, we accept most plans through the insurance companies on this page. Many patients, especially those who are seen infrequently or have high-deductible insurance plans, find the self-pay option to be less expensive than having our office submit claims to their insurance.

If your health insurance information is changing, please contact our office so that we can update your chart. We need active insurance information or a plan to do self pay in order for you to be seen for an appointment. Please provide us with your new health insurance information at least three days prior to your scheduled appointment.

If you have further questions about your plan, please call our office at 920.841.8326.

Options for Out of Network Insurance
You may still be able to be reimbursed for the cost of services by using your insurer’s Out-Of-Network (OON) benefits, even if your insurance company is not listed above. The process is usually straightforward but involves some paperwork on your part:
-
You pay the self pay cost at the time of service. Please review our self pay rates.
-
Farrar and Associates will provide you with a detailed receipt for your visit including billing codes for services you received.
-
Use the detailed receipt to file a claim with your insurance company. Procedures vary, but many insurance companies will allow you to file claims through a smartphone app or an online portal.
-
Your insurer reimburses you for a portion of the cost after you've met your OON deductible.
Please contact your insurance company to determine your level of OON benefits before seeking reimbursement. Farrar and Associates Mental Health does not assist with claims to Out of Network insurers beyond providing a detailed receipt.
