Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. All new patients must have a referral from their existing provider. Why do new patients need a referral?

 

New patient paperwork will be emailed to you when you schedule your appointment and can be filled out online ahead of your scheduled appointment. Alternately you can download the documents from the forms tab and print them yourself.

Please read the Patient Expectations document before your first visit. You'll be asked to sign a copy of this agreement before receiving care.

 

Please provide a list of all your medications and doses as well as proof of insurance. Contact your insurance company in advance to verify that Farrar and Associates has a contract with them.

 

Self-pay rates are available with full payment due at the time of service.
