Referral Process:
New patients seeking psychotherapy services or psychiatric care must first obtain a referral from a Primary Care Provider, OB/GYN, midwife, psychiatric prescriber, or therapist. Please review the exclusion criteria before submitting a referral.
Referrers should complete and fax the following to Farrar and Associates at 833.283.7571:

a release of information form for current or past psychiatric provider and PCP

relevant medical records:

notes pertaining to the patient's recent mental health issues

medication list

medical problem list

laboratory results from the last 6 months
