Referral Process:

 

New patients seeking psychiatric care must first obtain a referral from a Primary Care Provider, OB/GYN, midwife, psychiatric prescriber, or therapist.  Please review the exclusion criteria before submitting a referral.

Referrers should complete a referral form and fax the following to Farrar and Associates at 833.283.7571:

​​​​

• a release of information form for current or past psychiatric provider and PCP

• relevant medical records:

  • notes pertaining to the patient's recent mental health issues

  • medication list

  • medical problem list

  • laboratory results from the last 6 months
