The following requirements are based on what we know at this time as recommendations for best practices for health and safety:
• As of May 2021, Farrar and Associates is conducting appointments both in-person and via telehealth.
• If you would like to read our COVID-19 consent form ahead of time, you may read it here.
• When entering the office, please adhere to 6 feet of social distancing whenever possible. We have multiple hand sanitizer stations for your use.
• Masks are optional in common areas of the clinic.
• All of our clinicians and staff have been vaccinated against COVID-19.
• Farrar and Associates has the right to refuse to provide services if a client refuses to comply with any of the above noted requirements.
